Activity 14.2: Small Group Discussion on Filling the Out Patient Department Tally sheet and Register

DIVIDE students into small manageable groups (5-10 students).

ASK the student to read the scenario.

SCENARIO

Madeni Masogange Kamaliza is a 28 years old male weighing 55 Kg from Ngudu Village. He attended Malampaka health centre for the first time on 20/09/2013, with a history of periodic fever, headache and general body malaise for 4 days. He was tested for malaria using mRDT and the result was positive, Hemoglobin level was 12g/dl.

He was diagnosed to have uncomplicated malaria, Alu tablets (adult dose) and Tabs paracetamol 1000 mg tds for 3 days were given.

ASK the students to fill the above information in OPD register.

ALLOW group to work for 10 minutes and to fill the information from the scenario in OPD register then select one small group to present in plenary.

CONDUCT general discussion by asking other groups to contribute to the first group’s presentation.(15 Minutes)

SUMMARIZE the activity using training guidelines. (5 minutes)

Tally Sheet Form: This form is used concurrently with the OPD Register to ensure that information for OPD patients, which is reported at higher levels, is classified correctly and summed-up easily for purposes of reporting.

SCENARIO

OPD Register: This register records data for clients at the Out-Patient Department.

NB: The register should be filled out concurrently with the Tally Sheet Forms for accuracy of data. Start with the Tally Sheet Form.

Activity 14.3: Small Group Activity (15 minutes)

DIVIDE students into small groups (5-10 students)

DISTRIBUTE copies of the OPD Monthly report Form.

INSTRUCT students to look at the OPD Monthly report Form and follow along with the presentation

OPD Monthly Report Form: This form is filled out during the first week of the month following the reporting month. It is sent to the district but a copy remains at the facility.

OPD register has two important parts

Part one contains record of normal diseases

Part two records notifiable diseases (Epidemics) as guided by MoHCDGEC

Part that records normal diseases is at the front part and the part which records notifiable disease is at the end of the register

Notifiable part has two more variables; Date of commencement of the disease and Status of immunization

First Attendance: The visit by a patient to the OPD for the first time.

Each first attendance is given a number beginning each month with number ‘1’. The total number of first attendances determines the workload at the OPD.

Diagnoses: “Diagnosis” is the problem identified by a doctor from the patient whether confirmed in the laboratory or not. This problem may be an actual disease or related to another disease.

Long illness: A patient’s visit to a facility for the first time in a year, even with a long illness, should be regarded as first attendance. Subsequent visits, in that year, are referred to as return visits.

Return-visits

Patients carrying on with long term treatment (for example for management of tuberculosis or hypertension, chronic diarrhoea or Diabetes mellitus)

All return-visits will be tallied in the tally sheet as return visit

Amina Jumanne is a 28 years old male weighing 55 Kg from Ituha Village. He attended Kiwanjampaka health centre for the first time on 20/09/2016, with a history of periodic fever, headache, passage of loose stool (10 motions in the past 24 hours) and general body malaise for 4 days. He was tested for malaria using mRDT and the result was positive, Hemoglobin level was 12g/dl.

The stool appearance resembles that of Cholera

He was diagnosed to have uncomplicated malaria, Alu tablets (adult dose) and Tabs paracetamol 1000 mg tds for 3 days were given.

Mwajabu Kisomo Muya is a 35 years old female weighing 45 Kg from Kilimani Village. Shee attended Ruanda health centre which is in Mbeya City council, Mbeya for the first time in this year on 20/02/2017, with a history of periodic fever, vomiting, extreme body weakness and Joint pain for 4 days. On examination she was Pale. She was tested for malaria using mRDT and the result was positive, Hemoglobin level was 9g/dl.

She was diagnosed to have severe malaria with anaemia and was then admitted for further management

Report Form

The OPD Register has two parts, part one contains record of normal diseases while the other part records notifiable diseases (Epidemics) as guided by MoHCDGEC.

Notifiable disease have to be filled at the last part of OPD register

The facility in-charge should report to DMO immediately when an outbreak occurs
